The address entered in the repair order is the delivery and billing address. If you have different addresses, you can use the backside of the form.

If no email contact has taken place, please enter the email address legibly. Further contact is usually via e-mail. You can choose a shipping service provider for the delivery. The return shipment takes place in the insured package with DHL.
